Please complete the steps below to begin enrollment and ensure your child is fully registered and approved for care and meals.
1. Enrollment Application
Complete your child’s registration packet
with all requested information
2. Submit Required Documents
Email the following documents to HR@ziyadobasischool.com or drop them off in person to complete your child's file:

Up-to-Date Immunization Record

Most Recent Physical
3. CACFP Food Applcation
Submit the CACFP Food Application to
enroll your child in our free meal program
4. Confirmation + Next Steps
Once everything is submitted, you'll receive confirmation
along with any final steps needed to complete enrollment
